Continuum Care Partners was founded on a simple belief: patients in skilled nursing facilities deserve the same quality of specialty care as those in academic medical centers. Our nurse practitioners bring board-level PM&R expertise to the bedside — not just for the complex cases, but for every patient, every visit, without exception.

We are clinician-owned, patient-centered, and community-rooted. Every decision we make is guided by outcomes — for our patients, for our partner facilities, and for the healthcare system we are all responsible for improving.
OUR TEAM
Continuum Care Partners is built around a team of advanced practice registered nurses with specialized training in physical medicine, rehabilitation, neurosurgery, and chronic pain management. We chose the SNF setting not because it was easy, but because it's where the most vulnerable patients are — and where great NP care makes the greatest difference.

Our NPs are credentialed at each partner facility, carry active Medicare Part B enrollment, and complete continuing education in PM&R, CMS compliance, and procedural medicine annually.
